Your Hospital Stay
At Meeker Memorial, our staff is committed to ensuring you stay with us is comfortable. You can count on us to provide you with Care as it should be.
Registration Process
As part of the registrations and admitting process at Meeker Memorial you will be asked to complete and several forms including a consent form for admission and medical treatment.
Please bring the following information with you for your admission appointment:
- All current Health Insurance cards (if you have more than one insurance provider, bring information for each)
- Any paperwork or preparatory information given to you by your physician or that arrived in the mail ahead of time.
- Picture ID and Social Security Number.
- A parent or legal guardian for patients under 18 years of age.
If you have a Health Care Directive, Power of Attorney and/or Living Will you will also want to bring a copy as applicable. More information on Health Care Directives is available here.
Identification Band
During your stay at Meeker Memorial, you will be issued an identification band. This band will be placed on your wrist and will be removed when you are discharged. The information band will be used from identification and ensure you received the intended care.

MyChart Bedside
MyChart Bedside is an application on a tablet that patients receive during a hospital admission. It is intended to provide patients and families access to information about their health and treatment to make the hospital stay more predictable and productive. MyChart Bedside on inpatient units promote patient-centered care and help to engage patients and families.
